Home Medicines Review (HMR)

The Division is actively involved in the delivery of the Home Medicines Review (also previously known as the Domiciliary Medication Management Review) program in partnership with the Pharmacy Guild of Australia.

The main objective of HMR is to improve a patient’s understanding of their medications and thereby optimize benefits and health outcomes.

The HMR program promotes, educates and provides support by facilitating one-on-one visits, articles in the Division’s newsletters and group workshops with GPs, Pharmacists and Community Groups.

HMR provides an opportunity for the GP to view a “patient-medication relationship” in a context outside the patients visit to the surgery.
