Diabetes

The aim of this program is to support general practices/GPs to provide optimal care and contribute to the achievement of the best possible health outcomes for patients with diabetes. This includes best practice management of diabetes patients, early intervention and promotion of collaborative approaches to the care of these patients.

The program activities encourage a systematic approach to support general practices/GPs to provide optimal diabetes care by developing register, recall and reminder systems; and capturing and recording Aboriginal and/or Torres Strait Islander origin patients with diabetes. This part of the program includes the Division receiving de-identified electronic patient records to provide feedback on the quality improvement in diabetes care.

GPs are provided with current guidelines and recommendations for optimal diabetes management from Diabetes Australia and RACGP. The Division also collaborates with the local Area Health Services and other stakeholders to deliver improvements in patients HBa1c and total cholesterol.

The Division also maintains collaborations with other organisations and consumer/carer groups to assist the community to comply with the optimal approaches to diabetes care through enhanced relationships with business groups, local health services, local CALD groups, local Diabetes Australia Chapter and community clubs through Community Talks.
